Neck pain is a common complain, being in most cases due to non-thyroidal causes. However, a minority of patients suffer from painful thyroid diseases. Among them, sub-acute thyroiditis (SAT) is the most frequent one. Rare thyroid-related causes of neck pain include hemorrhage within a thyroid nodule as well as Riedel鈥檚 thyroiditis and suppurative thyroiditis. In the last 30 years, a few cases of a painful variant of Hashimoto鈥檚 thyroiditis (HT) have been described. Biochemical, ultrasound, and h...

To the Editor, Immunoglobulin G4 (IgG4) related thyroiditis is a newly defined disease characterized by extensive infiltration of lymphocytes and IgG4-positive plasmocytes, in combination with diffuse thyroid fibrosis, with or without elevated IgG4 serum levels [1]. Although subacute thyroiditis is the most common cause of thyroid pain, IgG4-related thyroiditis is regarded as a distinct diagnosis, with patients often experiencing severe discomfort. Despite differences in the pathology of the dis...

Hashimoto鈥檚 thyroiditis (HT) usually presents as painless thyroid swelling. Painful pediatric HT is a rare condition with limited literature on pain management. We report a 15-year-old female who presented with 4 weeks history of fatigue, malaise and progressive, painful midline thyroid swelling. There was no difficulty in swallowing, no fever or recent upper respiratory infection symptoms. Exam was remarkable for diffusely enlarged, very tender, and non-nodular thyroid. Thyroid function tests, ...

Thyroidal pain is usually due to subacute thyroiditis (SAT). In more severe forms prednisone doses up to 40鈥塵g daily for 2-3 weeks are recommended. Recurrences occur rarely and restoration of steroid treatment cures the disease. Rarely, patients with Hashimoto's thyroiditis (HT) have thyroidal pain (painful HT, PHT). Differently from SAT, occasional PHT patients showed no benefit from medical treatment so that thyroidectomy was necessary. We report three patients who did not show clinical respon...

Background: Hashimoto's thyroiditis (HT) is a common organ-specific autoimmune disease. Antithyroglobulin antibodies (TgAb) and antithyroperoxidase antibodies (TPOAb), predominantly of the immunoglobulin (Ig) G class, are hallmarks of HT. It has been reported that HT can be divided into IgG4 and non-IgG4 thyroiditis. The aim of our study was to investigate the meaning of this classification. Methods: Thyroid sections from 53 Hashimoto's patients with stored serum samples were collected to detect...

Background: An immune reconstitution inflammatory syndrome (IRIS) may complicate immune restoration following start of antiretroviral therapy (ART) in human immunodeficiency virus (HIV)鈥搃nfected patients. The occurrence of Graves' disease in the setting of an IRIS is well recognized. We hereby report a case of Hashimoto's thyroiditis, presenting as an acute painful thyroiditis, and as a complication of IRIS. Summary: A painful acute thyroiditis with thyrotoxicosis occurred in a 37-year-old HIV-i...
