Self-regulatory organizations (SROs) can be found in education, healthcare, and other not-for-profit sectors as well as in the accounting, financial, and legal professions. DeMarzo et al. (2005) show theoretically that SROs can create monopoly market power for their affiliated agents, but that governmental oversight, even if less efficient than oversight by the SRO, can largely offset the market power. We provide an experimental test of this conjecture. For carefully rationalized parameterizatio...

Noroviruses (NoVs) are increasingly being recognized as an important enteric pathogen of gastroenteritis worldwide. The prevalence of NoVs as a cause of diarrhea acquired by travelers in developing countries is not well known. We examined the prevalence and importance of NoV infection in three international traveler cohorts with diarrhea acquired in three developing regions of the world, Mexico, Guatemala, and India. We also characterized the demographics and symptoms associated with NoV diarrhe...

Collaborations of physicians and researchers with industry can provide valuable benefits to society, particularly in the translation of basic scientific discoveries to new therapies and products. Recent reports and news stories have, however, documented disturbing examples of relationships and practices that put at risk the integrity of medical research, the objectivity of professional education, the quality of patient care, the soundness of clinical practice guidelines, and the public's trust i...

A distributed health data network is a system that allows secure remote analysis of separate data sets, each comprising a different medical organization’s or health plan’s records. Distributed health data networks are currently being planned that could cover millions of people, permitting studies of comparative clinical effectiveness, best practices, diffusion of medical technologies, and quality of care. These networks could also support assessment of medical product safety and other public hea...

TO THE EDITOR: We would like to congratulate Fuccio and colleagues (1) on their well-performed meta-analysis comparing different durations of triple therapy for Helicobacter pylori infection. However, we disagree with their conclusion that prolonging treatment is unlikely to be a clinically useful strategy. In our opinion, this statement is not supported by their results. The authors confirm previous findings (2, 3) that extending duration of treatment from 7 days to 10 or 14 days improves eradi...

In 2007, breast cancer will have been diagnosed in more than 178 000 women in the United States, and more than 40 000 women will have died of breast cancer (1). Most of these women never had their risk for breast cancer assessed, and even fewer considered chemoprevention (2–5). Providing women with an estimate of their risk for breast cancer would provide an opportunity for them to consider options to decrease their risk. Women at low short-term risk for breast cancer may experience less anxiety...

Background: Researchers of sickle cell disease have traditionally used health care utilization as a proxy for pain and underlying vaso-occlusion. However, utilization may not completely reflect the amount of self-reported pain or acute, painful episodes (crises). Objective: To examine the prevalence of self-reported pain and the relationship among pain, crises, and utilization in adults with sickle cell disease. Design: Prospective cohort study. Setting: Academic and community practices in Virgi...
