A framework for human error analysis of offshore evacuations

A framework is presented to identify and evaluate the risks of human error for critical steps in the escape, evacuation and rescue (EER) process on offshore installations. A combination of expert judgment techniques and major incident investigations from industry were used to evaluate the risk for the evacuation stage.
