Abstract Background There is no systematic evidence review of the long-term results of surgical pulmonary valve replacement (PVR) dedicated to adults with repaired tetralogy of Fallot (rTOF) and pulmonary regurgitation. Methods Our primary objective was to determine whether PVR reduced long-term mortality in adults with rTOF compared to conservative therapy. Secondary objectives were to determine the postoperative incidence rate of death, the changes in functional capacity and in RV volumes and ...

Background There is growing awareness of the long-term impact of a Fontan circulation on the associated morbidity and mortality. Comparative data on the incidence of supraventricular arrhythmia and sinus node dysfunction following extracardiac conduit (EC) and lateral tunnel (LT) Fontans are controversial. We performed a meta-analysis pooling all available long-term results comparing the EC and LT Fontan, with a special focus on arrhythmia. Methods We performed a systematic search of PubMed, Emb...

Myocardial protection is essential in performing successful open heart surgery. Indeed, hypothermia, potassium induced arrest and ventricular offloading are major concepts that have improved surgical results in the last decades. The cold potassium-based cardioplegia given in a single induction dose followed by intervals doses have been adopted by many in their daily practice with good results. However, the ideal myocardial strategy remains elusive.

Abstract Background Complex aortic procedures are potentially associated with important blood loss and coagulopathy. The aim of this study was to assess the impact of rotational thromboelastometry (ROTEM, Tem International GmBH, Munich, Germany) on transfusion requirements after proximal aortic operations in a real-world setting. Methods This single-center retrospective analysis based on 385 consecutive patients undergoing cardiac surgeries involving the aortic root, ascending aorta, or aortic a...

Background: Contemporary outcomes following surgical management of thoracic aortic disease have improved; however, the impact of sex-related differences is poorly understood. Methods: A total of 1653 patients (498 (30.1%) female) underwent thoracic aortic surgery with hypothermic circulatory arrest between 2002 and 2017 in ten institutions of the Canadian Thoracic Aortic Collaborative. Outcomes of interest were in-hospital death, stroke and a modified Society of Thoracic Surgeons-defined composi...
